Not sure how a thread about poor shot placement leads to much. Yup we need be sure we take good shots and are capable with all weapons we choose to take afield. If you can't do those two things dont go. In no way am I saying this is the case with anyone on the thread, just a good general rule.

Possibly you had two very odd and unfortunate outcomes back to back. Maybe you can keep a 2" group to 50 yards with your bow, we dont know. As trapper says this possiblilty will always exist. It is our job to minimize the risk.

However, threads like these to me seem completely unecessary. If you have questions about poor shots and retrieval and lost game there are plenty of resources available, try google. Don't need more threads like these.
